    The Transforming Diabetes Programme aims to improve the quality of diabetes care across Buckinghamshire. Ensuring that all healthcare professionals have the right skills, confidence, competencies and support to deliver excellent diabetes care across Buckinghamshire is a key priority for NHS Aylesbury Vale and Chiltern CCGs. As part of this programme we would like healthcare professionals to complete the following short survey. The questionnaire will take less than ten minutes to complete and the data will be used by the CCGs to map the current skills and competencies that we have in diabetes care across the county and the areas for potential improvement and additional support.
